VCA and MCM Scholarship Acceptance
Congratulations on receiving a scholarship or award from the Faculty of VCA and MCM. Please note, unless advised otherwise, you have 4 weeks from the date on the offer letter to accept your award. Reimbursements, such as the FGSAG and SAF Music Provident Fund, must be submitted by the date advised in your email.
To accept your award:
Please complete only the documents requested by the Dean in your scholarship offer letter.
Step 1. Payment information (one only, use the method requested in your letter or email)
- Upload or update your bank details (current and graduating students) or
- New supplier form F04* (alumni or external parties) or
- Student supplier form F05/F06 (only if specifically requested, normally for reimbursements)
Part-time students are required to provide a Tax File Number (TFN) Declaration form as scholarship payments to part-time students are not exempt from income tax. TFN forms can be obtained from the Stop1 on the corner of Swanston and Grattan Streets, most newsagents or ordered online.
If required, scanned copies of the New supplier form, Student supplier form and TFN Declaration forms should be uploaded during Step 3.

Step 2. Thank you letter and bio
Your scholarship offer letter will indicate if this is required. The thank you letter should be uploaded during Step 3.
